Define your Guardian

Guardians must acknowledge their role in taking care of their family member or friend. They must appreciate and respect an individuals trust in them to be asked to be a Guardian. Guardians must embrace their role in facilitating potentially difficult conversations and decisions.

Outline Steps For Your Plan of Action

  1. Know that States have their specific laws/rules regarding legal documents and what needs to be included in writing. Please consult with your legal counsel regarding what is best for you.

Your Guardian Should Know You.

You should trust that they will carry out your Plan of Action and Honor your Advanced Directive.

Guardians should be someone that has your best interest in mind.

Durable Power of Attorney

This individual needs to be someone that you feel can carry out your instructions in the event you cannot speak for yourself.

**Note that you can specify Durable Power of Attorney specific to your HealthCare decisions

Advanced Directive

Advanced Directive gives you the opportunity to detail specific Medical Services that you wish to receive. This document will detail if and when you want life saving/sustaining medical services. GAH will provide samples of ADVANCED DIRECTIVES, recommend they should have discussion with their Medical Provider regarding establishing the Advanced Directive.

Durable Power of Attorney

  1. Identify who is your Durable Power of Attorney for Healthcare decisions and/or financial decisions. This is a discussion one should have with legal counsel.
  2. Verify that Durable Power of Attorney and Advanced Directive are signed and have been provided to appropriate parties.
    Original documents should be stored in safe place.

  3. The person you select as your Durable Power of Attorney needs to be someone who is aware of your Advanced Directive. They must be committed to insuring your directions are followed
